Create Checkboxes Forms in DForms

Step 1: Sign into Dforms and connect your wallet.

Step 2: The window will open up as shown in the screenshot attached below, click on the connect your wallet.

Step 3: Choose an account from which you want to login.

Step 4: The Dforms will open as shown in the below screenshot.

Step 5: Click on Create Form

Step 6: Click on the Checkboxes Option on the left-Hand side to start creating a checkboxes Dforms as shown below

Step 7: The window will appear to add details for making the “Checkboxes” form, add title and form Description
Add Questions and their Options.

Step 8: You can make the specific form field mandatory by enabling the “Required” tab.
So, whenever anyone will be filling the form, that specific question will be mandatory to answer otherwise he/ she can’t submit the form.

Step 9: Click on the Publish Tab on the top Left corner, you can also Preview the form before submitting it.
The Pop window will appear in which it will ask if you want to receive notifications, it will provide you with three options
1.Not to send any response
2. Notify on Form Submission
3.Notify with Form response

Step 10: Click on Publish and copy the Form Url and share with the person or Group from which you want to receive responses

Step 11: You can check the responses as shown below in the screenshot attached
