Create Multiple Choice Forms in DForms

Step 1: Sign into Dforms and connect your wallet.

Step 2: The window will open up as shown in the screenshot attached below, click on the connect your wallet.

Step 3: Choose an account from which you want to login.

Step 4: The Dforms will open as shown in the below screenshot.

Step 5: Click on Create Form.

Step 6: Click on the Multiple-Choice Option on the left-hand side to start creating a Multiple-choice Dforms as shown below

Step 7: The window will appear to add details for making the “Multiple Choice” form, add Title and Form Description.
Add Questions and their Options or choices
For example: – What is the capital of France? and write its options too like Paris, Berlin, Madrid and Rome. Screenshot attached below for better understanding.

Step 8: Click on the Publish Tab on the top Left corner, you can also Preview the form before submitting it.
The Pop window will appear in which it will ask if you want to receive notifications, it will provide you with three options
1.Not to send any response
2. Notify on Form Submission
3.Notify with Form response

Step 9: Click on Publish and copy the Form Url and share with the person or Group from which you want to receive responses

Step 10: You can check the responses as shown below in the screenshot attached

Step 11: Click on the Date written Below the submission Date, there you will get the option to view the response sheet, if needed can print and download it.
