Create Multiple Choice Grid Forms in DForms

Step 1: Sign into Dforms and connect your wallet.

Step 2: The window will open up as shown in the screenshot attached below, click on the connect your wallet

Step 3: Choose an account from which you want to login.

Step 4: The Dforms will open as shown in the below screenshot.

Step 5: Click on Create Form.

Step 6: Click on the Multiple-Choice Grid Option on the left-hand side to start creating a multiple-choice Dforms as shown below.

Step 7: The Window will appear add Details like Title, Form Description and the options in rows and column to make it a multiple-choice grid Dforms.
For example: –

• Add Rows (Items to be rated):
• Punctuality
• Staff Behaviour
• Service Quality
• Communication
• Value for Money

• Add Columns (Rating options):
• Excellent
• Very Poor
• Poor
• Average
• Good

Your form will look like this: –

Step 8: You can make the specific form field mandatory by enabling the “Required” tab by clicking on the three dots on right hand side.
So, whenever anyone will be filling the form, that specific question will be mandatory to answer otherwise he/ she can’t submit the form.

Step 9: Click on the Publish Tab on the top Left corner, you can also Preview the form before submitting it.
The Pop window will appear in which it will ask if you want to receive notifications, it will provide you with three options
1.Not to send any response
2. Notify on Form Submission
3.Notify with Form response

Step 10: Click on Publish and copy the Form Url and share with the person or Group from which you want to receive responses
