Creating Short Answer DForms

Step1: – Sign into D form and connect your wallet.

Step2: – The window will open up as shown in the screenshot attached below, click on the connect your wallet.

Step3: – choose an account from which you want to login.

Step4: – The D form will open as shown in the below screenshot.

Step5: – Click on Create Form

Step6: – Click on the Short answer Option on the left-Hand side to start creating a short answer D form as shown below

Step 7: – The window will appear to add details for making the “short answer” form, Add Questions, Title and Form Description.
You can customise it by changing the Font, making it bold changing into Italic and Underline as per your preference and then click on save the form will appear like this as shown below

Step 8: – Click on the Publish Tab on the top Left corner, you can also Preview the form before submitting it.
The Pop window will appear in which it will ask if you want to receive notifications, it will provide you with three options
1.Not to send any response
2. Notify on Form Submission
3.Notify with Form response

Step 9: – Click on Publish and copy the Form Url and share with the person or Group from which you want to receive responses

Step 10: – You can check the responses as shown below in the screenshot attached
