Creating Linear scale DForm

Step1: – Sign into D form and connect your wallet.

Step2: – The window will open up as shown in the screenshot attached below, click on the connect your wallet.

Step3: – choose an account from which you want to login.

Step4: – The D form will open as shown in the below screenshot.

Step5: – Click on Create Form

Step6: – Click on the Linear Scale Option on the left-hand side to start creating a Linear Scale D form as shown below

Step7: – The Window will appear add Details like Question, Form Description and the options for the Question to make it a linear scale Question
For example: – “How would you rate your overall satisfaction with out services?”
Scale:
1️⃣ – Very dissatisfied
2️⃣ – Somewhat dissatisfied
3️⃣ – Neutral
4️⃣ – Satisfied
5️⃣ – Very satisfied

Step 8: – You can make the specific form field mandatory by enabling the “Required” tab by clicking on the three dots on right hand side.
So, whenever anyone will be filling the form, that specific question will be mandatory to answer otherwise he/ she can’t submit the form.

Step 9: – Click on the Publish Tab on the top Left corner, you can also Preview the form before submitting it.
The Pop window will appear in which it will ask if you want to receive notifications, it will provide you with three options
1.Not to send any response
2. Notify on Form Submission
3.Notify with Form response

Step 10: – Click on Publish and copy the Form Url and share with the person or Group from which you want to receive responses
